Job Title: MN 245D Designated Manager/Coordinator

Job Description:

C.A Ellis & Associates, LLC is currently seeking an MN 245D Designated Manager/Coordinator to join our team. As a Designated Manager/Coordinator, you will be responsible for overseeing and coordinating services for individuals with developmental disabilities and related conditions in compliance with MN 245D regulations.

Responsibilities:

  • Ensure compliance with MN 245D regulations and guidelines.
  • Develop and implement person-centered service plans for each individual.
  • Monitor and evaluate ongoing training and development.
  • Coordinate and communicate with families, guardians, and other stakeholders to ensure individual needs are being met.
  • Maintain accurate and complete records and documentation.
  • Prepare reports as required by regulatory agencies.
  • Enforcing policies and procedures
  • Conduct client intake, admission and assessments.
  • Attend client Support Team meetings as needed
  • Develop and implement client CSSP-A’s
  • Review client documents so that they meet licensing guidelines (Annual Outcomes and Progress Reviews, and other related reports), keeping a working knowledge of regulatory requirements.
  • Maintain accurate records and reports.
  • Serve as a communication liaison with guardians and other service providers.
  • Review incident and emergency reports, identification of incident patterns and implementation of correctives actions
  • Serves as a client advocate
  • Conduct program evaluations and quality assurance reviews

Requirements:

    • 2-year degree in a human service field and a minimum of 2 years of direct care experience
    • Strong leadership and communication skills.
    • Knowledge of states and federal requirements as they pertain to the 245D regulations.
    • Ability to work independently and as part of a team.
    • Must pass a background check.
    • Minimum of 4 years of direct care experience under the supervision of a Designated Manager
    • Minimum of 3 years of progressively responsible supervisory and personnel management experience in Human Services.
    • Ability to develop, implement and evaluate client programming.
    • Strong written and oral skills

    Preferred:

    • Experience with completing 6790s and Companion Report
    • Work independently
