Job Description:
C.A Ellis & Associates, LLC is currently seeking an MN 245D Designated Manager/Coordinator to join our team. As a Designated Manager/Coordinator, you will be responsible for overseeing and coordinating services for individuals with developmental disabilities and related conditions in compliance with MN 245D regulations.
Responsibilities:
- Ensure compliance with MN 245D regulations and guidelines.
- Develop and implement person-centered service plans for each individual.
- Monitor and evaluate ongoing training and development.
- Coordinate and communicate with families, guardians, and other stakeholders to ensure individual needs are being met.
- Maintain accurate and complete records and documentation.
- Prepare reports as required by regulatory agencies.
- Enforcing policies and procedures
- Conduct client intake, admission and assessments.
- Attend client Support Team meetings as needed
- Develop and implement client CSSP-A’s
- Review client documents so that they meet licensing guidelines (Annual Outcomes and Progress Reviews, and other related reports), keeping a working knowledge of regulatory requirements.
- Maintain accurate records and reports.
- Serve as a communication liaison with guardians and other service providers.
- Review incident and emergency reports, identification of incident patterns and implementation of correctives actions
- Serves as a client advocate
- Conduct program evaluations and quality assurance reviews
Requirements:
- 2-year degree in a human service field and a minimum of 2 years of direct care experience
- Strong leadership and communication skills.
- Knowledge of states and federal requirements as they pertain to the 245D regulations.
- Ability to work independently and as part of a team.
- Must pass a background check.
- Minimum of 4 years of direct care experience under the supervision of a Designated Manager
- Minimum of 3 years of progressively responsible supervisory and personnel management experience in Human Services.
- Ability to develop, implement and evaluate client programming.
- Strong written and oral skills
Preferred:
- Experience with completing 6790s and Companion Report
- Work independently
